The following is a list of Health and Welfare plan booklets and plan booklet amendment inserts that can be requested from the Fund Office.

Some of the plan documents may be available for online viewing in the PDF file format. Adobe's Acrobat Reader® is required.

Pension benefits information, rules and regulations.
(1) Improved Regular Pension formula; (2) “Extra” benefit check issued; and (3) Temporary Supplemental Benefit extended.
Grace period allowance for failure to work minimum hours in Covered Employment in a Plan Credit Year.
New optional payment methods for the Husband-and-Wife Pension.

Extension of the Temporary Supplemental Benefit.
New procedures for filing or appealing claims for benefit coverage (effective 01/01/2002)
Changes to the Plan's crediting factor for Contributions for Hours Worked.
(1) Change in plan definition of “Total Disability”; and (2) change to the “Credited Service During Periods of Disability” (both effective August 1, 2003).
Extension of the Temporary Supplemental Benefit (effective December 1, 2004)
Changes to the Surviving Spouse Pension (effective January 1, 2005)
Changes to Permanent Break in Service Rules (effective March 1, 2005)
