Laborers Trust Funds for Northern California
has four medical plan options for eligible Participants, based on their current status:
Laborers
Direct Payment Plan
A Preferred
Provider Organization (PPO) plan which uses the Anthem
Blue Cross Prudent Buyer Plan’s network
of participating hospitals, physicians and ancillary service
providers who have agreed to provide medical care to plan
participants at reduced rates.
Health Net
A Health Maintenance Organization (HMO) plan which provides two
comprehensive medical and health benefit services options only to our
Retiree participants: Health Net Regular for Non-Medicare Retiree ( “Regular Retiree”
) participants and Health Net “Seniority Plus” for Medicare-eligible Retiree ( “Medicare Risk”
) participants.
PacifiCare
HMO plan which provides two comprehensive medical and health benefit
services options only to our Retiree participants:
PacifiCare Regular for Non-Medicare Retiree ( “Regular Retiree” ) participants and “Secure
Horizons” for Medicare-eligible Retiree ( “Medicare Risk” ) participants.
If you live
in an area which does not contain an HMO program, if eligible,
you will be covered by the Laborers Direct Payment Plan.
